Can we add this line on an Online Donation form?

Probably not anywhere that would flow correctly on the form. But you can easily add that line of text on the page where the form resides!

You could also add that to the email receipt.

Thanks for the replies....now the customer is stating this "Gift preferences show on top and include multiple fields. The second screenshot is our current page with dropdown for reasons donors came to website and I would like to have something similar."
