Forms library - Field choices different

In trying to create a requirements form, the field choices are different when going to Blueprint and creating a new form then when creating a new form from Forms Manager within Applications. 

The field choices are much fewer there then witin Blueprint.  Why is there a difference and can I create a form in Blueprint, activate it and have it show up in Applications, Form Manager?
