How does the email approval process work in the real world?

I have three questions around email approval, and how it works in the "real world" for Convio users:

1) If you create a Reviewer List as part of the audience for a message, what do the messages look like that those persons receive? Are the identical to the message the constituent will receive? Is the personal and conditional content tailored for the reviewer? Is there a special subject line or header that alerts the reviewer that this a message that needs their review? Are there instructions as to what to do or who to contact if the reviewer has feedback?

2) If you are an admin who has rights to create a message, but not approve it: a) what is the best way to notify the approver that there is a message waiting for them to approve? b) what steps does the approver need to take? Does he or she need to sign in to the admin area and click the "Approve" button on the message?

3) I'm curious to know how many folks make use of the Reviewer list, as opposed to the "Send a test message" option.
