Donation form issues (donor being sent back to enter info again)

We've had a few donors with issues when attempting to donate. They fill out all information, please 'donate now' then the page hangs for a moment, then sends them back to enter in their gift information a second time instead of to the gift confirmation screen. This results in 2 gifts being made. We've also noticed those gifts are being lumped together in RE (i/e two $50 gifts shown as 1 $100 gift receipted).

This type of question is probably not possible to answer on the forms. I recommend to create a case so and analyst can review this.
