NetCommunity email prompt from user

I wonder if anyone has any ideas about how we can create a page/form in our NetCommunity that sends us an email, which is linked to the logged-in user.



We will be issuing unique codes to purchase an NUS card upon request, and would like this to be restricted to those registered (and encourage others to register), but not require them to complete another form with their details we would require for verification of their eligibility.



We have thought about using an attribute which will be updated to Raiser's Edge, but this would require us to run regular queries to determine who has made the request, which would delay our intended speedy response.
